Integrated care is the here, the now and the future of delivering care that is joined up and person-centred, but the scale and complexity of this collective ambition presents a real challenge for health and social care system leaders.
Our Integrating Care team was asked by the Local Government Association (LGA) to develop a practical support package for local authority and health leaders to help with the successful integration of services. The focus of the work is to ensure that local areas have the relevant tools and the understanding of what has worked successfully in other areas, to help them develop integrated care effectively, reducing the fragmentation or duplication of care.
The project has been largely successful so far, my role being focused on the development of the 11 value cases summaries of areas of best practice, 4 of which have been published so far. These value cases were extremely interesting to research and gave me the invaluable experience of being able to co-create with a number of different stakeholders, that are at the forefront of integrated care. These individuals make up some of our Community of Practice that spans nearly 200 organisations with stakeholders from CCGS, LA’s, DH, public health, hospitals, and charities.
We particularly enjoyed working with Helmut Hildebrandt, the CEO OptiMedis AG & CEO Gesundes Kinzigtal GmbH, who has done some excellent work in Kinzigtal Germany using an integrated population-based approach. The development of Gesundes Kinzigtal has resulted in great outcomes and provides some beneficial lessons that we can apply here in the UK.
I for one now have a much better understanding of the different initiatives that are taking part across the country and the best practice for the integration of care, and as a result of our work local areas will too.
