The challenge

Realising the Value was part of a programme of work with Nesta, the Health Foundation, NHS England and a range of other partners to enable people to take a more active role in their healthcare, improving the quality and sustainability of services for those living with one or more long term conditions.

Our approach

An 18-month programme was established to support the NHS to bring together a shared understanding of person- and community-centred approaches to health and wellbeing, and make specific recommendations on how they could be applied to have maximum impact,

Together with colleagues from Nesta, the Health Foundation and NHS England the PPL team worked with sites across the UK, and partners including Being Well Salford and Positively UK, to build the case for scaling person and community-centred care.

Our products included joint development with the NHS England Finance Team of economic modelling tools used by commissioners across England to quantify the impact and prioritise investment in this area.

Our impact

For peer support and self-management alone, direct financial savings were identified about £2,100 per person, £5.2m per commissioning area, or £950m nationally.

These formed the basis of both further strategic planning and investment both locally and nationally.
