The challenge

Hampshire and the Isle of Wight has a reputation as a forward thinking and innovative region. In recognition of their ambitions, Hampshire’s Better Local Care programme was selected as a Vanguard site by NHS England to test new ideas and share learning across the country. Within the Vanguard, the Fareham and Gosport area was identified as an Accelerator site, with learning from the locality expected to inform the development of integrated care across the Hampshire and Isle of Wight.

A key reason that Fareham and Gosport was chosen as an Accelerator site was to stem the mounting pressure the health and care system was facing. Some of the more significant issues included rising demand, meeting key performance targets, staffing and providing appropriate access.

Our approach

The PPL team worked with Fareham and Gosport as well as partners at OptiMedis-COBIC (OMC), creators of one the world’s best documented and most effective integrated care systems, Gesundes Kinzigtal. The work was based around the ‘The Quadruple Aim’ of:

  • improved population health
  • improved quality and experience of care
  • reduced cost per case
  • increased staff satisfaction at work
Our impact

During a year, PPL, OMC and partners were able to help the local system develop a system integrator approach, designing and implementing processes to adapt to the needs of local providers and communities, and sharing the lessons learnt far beyond. Practical changes have included:

  • providing the tools and data for practices to identify at-risk patients and measure system success
  • creating a joint collaboration agreement and forums for providers and commissioners to plan care for their population together​
