Realising the Value of Person-Cented Care
Realising the Value was an 18-month programme set up to support the NHS to bring together understanding of person- and community-centred approaches to health and wellbeing and make specific recommendations on how they could be applied to have maximum impact.
Together with colleagues from Nesta, the Health Foundation and NHS England we worked with sites across the UK, and partners including Being Well Salford and Positively UK, to build the case for scaling person and community-centred care.
Our work included joint development with the NHS England Finance Team of economic modelling tools used by commissioners across England. For peer support and self-management alone, direct financial savings of about £2,100 per person, £5.2m per CCG, or £950m nationally were modelled and agreed.
